Plumbing Applications can be dropped off or mailed to 310 S Main St Bellefontaine OH 43311 or Email to Plumbing@loganhealthohio.gov. Must be paid in full with check or card to be process application. We accept card over the phone as well for 3% fee.

There are two links, one is for both commercial and residential plumbers and one is for commercial only. They can both do the same work for residential, and one for commercial only is for Commercial plumbers that have a state license which are the only people who can work on commercial properties or businesses.
